A medical backpack is a relatively compact and easy-to-transport set of medical equipment for cars, tourism and active leisure.

I hope you remember that the Blow-Out Kit is the minimum medical supplies needed to counter the three most common brutal causes of death, which we have covered in a separate publication. Having with you first-aid kits such as Cargo Pocket FAK or Blow-Out Kit, you will quickly begin to realize that there are not enough components in their composition to help with domestic health problems, dislocations or fractures, serious injuries. This is a rather expected course of events, as these kits are far from universal and comprehensive.

Along with this, you will notice that if you want to add multiple components to your trauma first aid kit, you will quickly run into the problem of overall kit sizes. Adding only splints, hygiene products and pills for common household health conditions almost doubles the volume of the first aid kit, if you want to add specialized equipment - you go beyond the pouches. They become too oversized for comfortable transportation and movement with them. I am not a fan of bags, as they severely restrict movement and fast movement, so I would advise you to pay attention to a slinger like the 5.11 UCR Slingpack or a backpack.

Below I will show a medical backpack, which will become the basis for provision for cars going out at the shooting range, hunting, fishing, tourism, long hikes and active leisure for a group of up to four people.

Important!

Like any set of equipment, this medical backpack is built with the purpose and the level of training in mind. I am not a medical qualification, so the kit does not include means for carrying out purely medical manipulations: intubation, conicotomy, organization of intramuscular and intravenous access, administration of medications, infusion therapy, suturing, etc. But the content is intended to provide pre-medical care for injuries and injuries, basic life support and organization of evacuation in conditions of distance from civilization, and lengthening the time until the arrival of qualified medical personnel.

Backpack and appearance

Since the selected set of equipment is significantly larger than the Blow-Out Kit, the following items of equipment were selected for transportation:

- TSSI TACOPS M-9 Assault Medical Backpack in Ranger Green color as the main container for medical supplies.
- Pouch Condor EMG Glove Pouch in OD color for external placement of nitrile gloves, which are used during examination of victims.
- Pouch MSM EMT Trauma Shears Pouch in OD color to accommodate atraumatic medical scissors, which are used to cut clothes and equipment during the initial examination for injuries and damage.
- PVC patch "MED" in Black color for contrasting designation of the medical direction of the backpack, visual difference against the background of the rest of the equipment.

Medical backpack

In front and on the sides, the medical backpack holds the equipment necessary for the initial examination of the victim in order to understand the means you will need in the future. If critical bleeding is detected during an external examination, tourniquets are also fixed to stop them, which will allow you to reduce the amount of blood loss, and buy some time to complete the examination and remove the rest of the tool from the inside of the backpack. Here are the items listed below.

- Harness turnstile NAR Combat Application Tourniquet (C-A-T) Gen7 4 pcs. - a means of stopping bleeding of the extremities. They are externally secured with shock cord loops for quicker access in case of critical bleeding and temporarily stopping moderate bleeding while you open the backpack or search for appropriate tools.
- Nitrile examination gloves, 10 pcs. - five pairs of heavy blue nitrile medical gloves. On blue gloves, in contrast to black ones, blood stains are better visible during the initial examination of the victim, especially in limited lighting conditions.
- Medical scissors MSM EMT Trauma Shears 1 pc. - atraumatic scissors with blunt ends for cutting clothes and access to the lesion site. They also help with opening the packaging of medical products.
- Soft evacuation clothes 1 pc. - textile weights, which have six handles, and are used to transport the victim over relatively short distances. They are located outside and are fixed with the lower compression straps of the backpack so as not to waste useful space inside.

At the back, in its flat pocket on the back, the medical backpack contains flat items, the overall dimensions of which take up a lot of space inside, are removed first or last, depending on the results of the initial examination. The following funds are located here.

- Neck collar Ambu Perfit ACE Military 1 pc. - an immobilization agent for the cervical spine. A very relevant thing in case of road accidents, falls and diving from a height. It is removed first of all when a neck injury is detected or suspected.
- Chemical heaters 4 pcs. - bags that generate heat by oxidation. Well suited for warming hands and feet at low temperatures, warming an injured person or counteracting the development of hypothermia caused by blood loss.
- NDūR Emergency Survival Blankets 2 pcs. - high density mylar safety blankets that block the effects of precipitation, convection and help maintain thermoregulation. Used together with chemical warmers to counteract hypothermia, and are applied after examination and targeted assistance.

Medical backpack

Before starting the examination or treatment, you must first approach the victim safely. In the conditions of tourism, hiking, skiing or snowboarding, outdoor activities or an accident, they may be far from the safest places for inspection.

In the lower outer pocket of the TSSI TACOPS M-9 Assault Medical Backpack for such cases, there is a set of equipment for safe approach to the victim, descent or ascent for inclined surfaces. It includes the following components.

- Edge Kit using Petzl Micro Traxion, Petzl Tibloc, Petzl OK carabiners and Tendon ropes - a compact system for fixing in close proximity to the edge and on slopes. Allows you to organize a personal anchor point to reach the victim, or lift the victim up an inclined surface. TSSI TACOPS M-9 Assault Medical Backpack pockets are used to categorize and better organize these groups. - Nitrile examination gloves, 10 pcs. - five pairs of heavy blue nitrile medical gloves. They are located from the outside in a separate pouch to speed up access.
- Safety glasses 3M SureFit SF201AF-EU 1 pc. - compact and lightweight protective goggles with anti-scratch and anti-fog coatings. Prevent the entry of infectious diseases through the mucous membrane of the eyes.
- Protective medical masks 2 pcs. - spare disposable medical masks for protection against infectious diseases transmitted by airborne droplets.
- A bottle of alcohol-based Sanitizer 1 pc. - an antiseptic agent for cleaning the surface of the hands after examination, or when changing pairs of gloves between reviews of various victims.
- Paramedic gloves 2 pcs. - a pair of high density nitrile gloves. Well suited for hand protection during evacuation (holding and carrying, etc.) and performing activities in which ordinary gloves are easily damaged.
- Gloves NAR Black Talon Gloves 2 pcs. - a pair of "tactical" nitrile gloves of black color in the case when blue can unmask the wearer.
- Headlamp Petzl e + LITE 1 pc. - spare flashlight in case of absence or failure of the main lighting means. Has modes of white and red light with two levels of intensity for examination and assistance in the dark. Also flash modes in white and red for alarming and marking its position during evacuation.

This group includes tourniquets and dressings for stopping critical bleeding in limbs and areas where tourniquets are not used.

- Hemostatic bandage Z-Medica Quikclot Combat Gauze Z-Fold 2 pcs. - to block critical bleeding in case of penetrating lesions, packing wounds where the use of tourniquets is impossible or impractical. Now there are 2 pieces, but it is planned to increase the number to four.
- FirstCare Emergency Bandage 4 pcs. - the main dressing material for blocking critical bleeding, the imposition of compression bandages instead of or together with tourniquets.
- Bandage NAR Compressed Gauze 4 pcs. - sterile cotton bandage with high absorption for plugging penetrating and deep wounds, stopping bleeding where the use of tourniquets is impossible or impractical.

Aids for injuries and injuries

- Plaster 3M Steri-Strip Reinforced Adhesive Skin Closures 4 pcs. - the so-called "patch sutures", adhesive strips for collecting the edges of the wound, can replace surgical sutures at the prehospital stage.
- Plaster for dry calluses 5 pcs. - volumetric plaster for dry calluses. Now I am looking for something more similar to the free-form Moleskine, since calluses are one of the most common types of leg injuries in the field.
- NAR Polycarbonate Eye Shield 4 pcs. - polymer pads to protect damaged tissues in case of trauma and penetrating eye injuries.
- Napkins Dynarex Sterile Gauze Pads 4 × 4 "12 Ply 12 pcs. - Sterile twelve-ball gauze napkins measuring 10x10 cm in individual packaging for treating minor and superficial injuries.
- Burn stickers Kikgel Burntec 4 pcs. - hydrogel stickers with dimensions of 10x10 centimeters for combating burns of 1-3 degrees and accelerating their healing.
- Elastic bandage 8x200 cm. 4 pcs. - auxiliary dressing material for compression bandages, creating additional pressure on the Quikclot Combat Gauze Z-Fold floor, fixing joints during sprains, dislocations, and the like.

Aids to maintain airway and breathing

In qualified medical personnel, airway management and breathing support are usually two separate groups with a large number of specialized equipment for performing medical procedures: intubation, conicotomy, oxygen administration, and the like. In the absence of medical qualifications, in my case, this group includes only means to counteract the development of pneumothorax and mechanical support of the upper respiratory tract, namely:

- Ventilator bag Micro BVM Pocket BVM 1 pc. - a professional tool for carrying out artificial lung ventilation (ALV) and supporting the victim's breathing.
- Film-valve for CPR 2 pcs. - protection against infectious diseases and contact with body fluids of the victim during cardiopulmonary resuscitation (CPR). A spare means in the ventilator bag in the case when it is already occupied, or the number of victims requiring ventilation exceeds each other.
- NAR ARS Needle Decompression Kit 2 pcs. - to combat closed / tense pneumothorax. The placement of a decompression needle is a medical manipulation for which I am not qualified, so I am not allowed to use them on victims. However, I prefer to have them as part of this kit in case of need and presence of appropriately qualified personnel.
- Instruction for NAR ARS Needle Decompression Kit 2 pcs. - retained for illustration with the installation site. As a reminder for medical personnel in case of decompression on me.
- Nasopharyngeal air duct Rüsch 2 pcs. - to restore patency of the upper respiratory tract, carrying out artificial ventilation on the injured in consciousness.
- Lubricant for air ducts 2 pcs. - to facilitate the installation of the air duct in the victim's nasal throat.
- Oropharyngeal air duct 9 pcs. - a set of air ducts ranging in size from 4 to 12 to restore the patency of the upper respiratory tract, artificial ventilation on victims in an unconscious state.
- Occlusive plaster NAR HyFin Vent Chest Seal 4 pcs. - a ventilated version of the North American Rescue (NAR) HyFin Chest Seal occlusive patch. Due to the presence of channels that act as valves, it prevents the development of closed / tense pneumothorax. Allows air to leave space in the pleural space during exhalation, and at the same time block the flow of air during inhalation.

In this group, the medical backpack includes splints, dressings and bandages to immobilize injured limbs. Now these are the following tools.

- Flexible tire SAM Splint February 2 pcs. - a tire with an aluminum base and a damping layer on both sides. It can be used as a rigid splint for fractures of the upper and lower extremities, as well as an improvised cervical collar, an improvised splint for the pelvis, and the like.
- A set of cards of the victim 3 pcs. - a standardized method of recording information about the victim, his injuries and the provided pre-medical assistance. Serves as a source of information when transferring the victim to higher levels of medical support.
- Autoadhesive bandage 3M Coban Self-Adherent Wrap 4 pcs. - a dressing whose surface adheres to itself but does not adhere to other surfaces. In combination with a flexible splint, it allows to achieve fixation of the limb at the level of the plaster, and at the same time does not damage the damper layer of the splint, as does an adhesive tape or a plaster. It is also actively used on pets, on whose wool ordinary bandages do not hold well.
- NAR Triangular Bandage 2 pcs. - a triangular bandage for fixing injured upper limbs, protecting large surfaces, immobilizing and making suspensions, etc. It can also be used as an impromptu tourniquet to stop bleeding in the extremities.

Means of collecting vital signs

This group includes devices and means of recording the vital signs of the victim for the subsequent transfer of this data to the search and rescue services and medical personnel. Without medical qualifications, you cannot use them to make decisions about further assistance, medication or treatment, but you can transfer them to the higher levels of medical support using a telephone or radio station. Accurate vital signs allow them to prepare in advance to provide appropriate care. The dynamics of indicators is important for diagnosing the development of shock or a possible deterioration in the victim's condition, and timely informing rescuers about them. This group includes the following devices.

- Tonometer Gamma 700K 1 pc. - an affordable mechanical method for collecting information about blood pressure. Provides high accuracy and does not require batteries.
- Pulse oximeter Boxym oSport 1 pc. - a device for collecting data on heart rate and saturation. Allows you to assess the quality of blood and oxygen circulation in the victim's body, as well as monitor their change before the arrival of qualified personnel.
- Thermometer Gamma Thermo Soft 1 pc. - electronic thermometer for collecting information about body temperature. It will help with the diagnosis of domestic health disorders during the field trip, and together with life blankets and chemical heaters, help control the restoration of the body's thermoregulation in case of hypothermia.
- USB cable - USB-TypeC 1 pc. - to recharge the pulse oximeter.
- Batteries LR41 2 pcs. - a set of replaceable batteries for the thermometer.
- Batteries CR2032 2 pcs. - a set of replaceable batteries for the Petzl e + LITE flashlight.

Important!

If you decide to add vital signs to your medical kit, be sure to learn how to use them and also become familiar with the conditions that may affect their readings. Otherwise, the data you collect may not be accurate.
